APPLICATION INSTRUCTIONS
FOR RESOURCE LEASE, PERMIT OR OTHER ENTITLEMENT FOR USE
(GEOTHERMAL, MINERAL, OR OIL & GAS)
The attached Application Form was developed in accordance with Government Code Section
95940. The form was designed to apply to a variety of land use situations including surface
and subsurface resource development involving small and large acreages. The form is
intended to permit the applicant to describe fully the proposed use of the State lands or
resources.
The information sought in the Application Form is required from the applicant and the
sufficiency of the information provided by the applicant will be the basis by which the staff will
determine the completeness of the application as specified in Government Code Section
95943.
When completing the Application Form, please type or write clearly, answer all questions and
write "NA" if a question does not apply. The information and material requested are
necessary to process your application. In addition, please submit any information believed
important in support of the application.
All plans and materials submitted become part of the official file and cannot be returned;
however, certain information deemed proprietary by statute may be withheld from public
review if requested by the applicant. You have the right to review the files maintained about
you, except as provided by law. Questions regarding the files should be directed to the

Failure of the applicant to adequately complete the Application Form and provide the
information and material requested within the time limits prescribed by the staff may result in
significant delays in determining the application complete or may result in the cancellation of
the application.
Public Resources Code Sections 6000, et seq. and Title 2, Division 3, Sections 1900, et seq.
of the California Code of Regulations govern the conduct of the Commission. The provisions,
by reference, are included herein.
