                        FOR JUDICIAL CANDIDATES FOR
                       THE SUPREME COURT OF MISSOURI
                       OFFICE OF GOVERNOR MATT BLUNT

Please answer the questions in this document as completely and fully as you can. You may
attach anything you feel relevant. Other than personal information, you should expect that the
completed questionnaire and any attachments will be public. We understand that all information
sought may not be readily available. Please make your best effort. Where written materials are
requested but not available, please summarize them as best you can.

      Please err on the side of providing too much detail rather than too little. Please include
      military service and provide the type of discharge for any completed service.

16.   Provide a detailed summary of all groups and organizations of which you are now a
      member or have been a member in the last 15 years. For each organization, provide the
      name of the organization, dates of membership, any offices or leadership positions you
      hold or held, and a summary of the group's mission or purpose.

17.   Have you or your spouse ever been a member of or worked for any organization that
      discriminates or discriminated on the basis of race, color, religion, national origin,
      ancestry, sex, disability, or familial status? If so, explain these policies and any actions
      you took to address them.

18.   Have you prepared, participated in the preparation of, or permitted use of your name on
      any reports, memoranda, or policy statements in connection with any public office? If
      yes, provide copies.

19.   For every lawsuit in which you have been a party, please provide the following: (a) list
      of all parties, including their designation as plaintiff, defendant, etc.; (b) copies of all
      complaints or petitions, answers, sworn testimony (written or oral), and dispositive
      orders and judgments; and (c) the style of the case, court (trial and appellate), and case
      numbers. If copies of the requested documents are not reasonably available, please
      summarize the facts, issues, and disposition of the case.

      your positions among different types of courts.

43.   Describe your position on compensation of Missouri judges.

44.   Describe your position on the scope of the Missouri Supreme Court's constitutional
      authority to superintend lower courts.

45.   Describe what you consider the most significant administrative issue facing the
      Missouri courts and provide your solution.

46.   Provide copies of all documents filed by you with the Missouri Ethics Commission or
      the Federal Election Commission or any campaign committee for which you were the
      candidate.
