MU STUDY ABROAD DEPARTMENTAL PROGRAMS
-PROGRAM CONFIRMATION FORM AND
CONDITIONS OF PARTICIPATION
As a participant in an MU study abroad program, please review and sign the following statement of your
obligations:
1. I confirm my intention to participate on a departmental study abroad program by returning this signed form
to CAFNR International Studies. I understand that this acceptance
is a firm commitment to participate in this program. If an emergency makes it impossible for me to
participate, I will notify my departmental program coordinator and the International Center immediately in
writing.
2. I realize that I must be in good academic and disciplinary standing (as defined by my school or college)
and must have successfully completed all program pre-requisites by the time of departure or I may be
withdrawn from the program.
3. In order to ensure that all study abroad participants enrolled at MU are covered by comprehensive health
insurance while abroad, MU requires all participants to verify that they have comprehensive health
insurance that will cover them overseas as well as emergency travel health insurance. The latter is
provided by the International Student Identity Card (ISIC). If I find that my current health insurance does
not provide comprehensive health insurance coverage while I am abroad, I agree to purchase the MU
Sickness and Accident Policy for Study Abroad offered through HTH Worldwide.
4. In the event of sickness or injury, I authorize the MU contact person at my host institution abroad to secure
whatever treatment is deemed necessary, including the admission to a hospital, the administration of
anesthetics, the transfusion of blood and surgery.
5. I have read the University of Missouri Assumption of Risk and Release Form and understand that failure to
submit this form will result in termination of my participation in study abroad.
6. Financial Aid: I understand that the International Center does not administer any financial aid. If I wish to
receive any financial aid (scholarships, grants, loans, etc.) for my study abroad program, I must contact
the Financial Aid Advisor for Study Abroad (11 Jesse Hall, 882-7506). I must submit all necessary
documents before I leave the United States. The International Center cannot fill out any paperwork for
me in my absence.
7. I am aware that University of Missouri students on MU-sponsored programs are required to make program
payments according to the schedule of payment outlined by the University of Missouri and the overseas
university, and that any outstanding charges will be billed to my University of Missouri account.
8. I understand that I will be refunded only costs that can be recovered by MU if I choose to withdraw from a
study abroad program before its conclusion.
9. In the unlikely event that the study abroad program in which I am participating is cancelled due to
circumstances beyond the control of the University of Missouri-Columbia, it is not guaranteed that I will
receive full academic credit for the program. In addition, I may not receive a full refund of program costs.
10. Recreational travel must not interfere with regularly scheduled courses.
11. If I choose to travel prior to or during the program, I understand that the University of Missouri-Columbia is
not responsible for me during non-program related travel. I understand that any risky activity or travel in
which I choose to become involved outside of the programs will be at my own expense and risk. While
MU employees may provide information regarding extra-curricular activities or travel, in no way does this
represent the University of Missouri-Columbia's endorsement of those activities or destinations.
12. I realize that I am expected to observe and not to participate in any political activities while abroad.
Becoming a political activist may endanger my own safety or that of my colleagues in the program, and will
almost certainly jeopardize the program itself.

13. I agree that the MU Director of Study Abroad and/or Program Director may terminate my participation in
the program if: 1) I engage in actions endangering to myself or others, or 2) my conduct is considered to
be detrimental or incompatible with the best interest and welfare of the program. I further agree, if expelled
from the program, to be responsible for all expenses incurred in returning to the United States.
14. I understand that it is my responsibility to make all travel arrangements and obtain my passport and
student visa.
15. I realize that attendance at the general orientation sessions is mandatory for all MU students participating
in study abroad programs and my absence may result in termination of my participation in the program.
16. I agree to complete all program-related forms, including supplementary applications materials required for
my program, and program evaluation forms issued by the University of Missouri International Center &
CAFNR.
