·   Promo Period: August 11 to 31, 2008
·   Go to http://www.promoprocess.com to register online and to print the Rebate Claim
    Form.
·   Sign and date the Rebate Claim Form and mail with the following:
       · a copy of the Invoice or Receipt (packing slip/order confirmation not accepted)
       · original UPC Code, Product Code and Serial No. labels from product box


Terms and Conditions:
·   Under NO circumstances Photocopies of UPC will be accepted.
·   Your request must be postmarked within thirty (30) days of Invoice Date. Promo Process is not responsible for lost,
    destroyed, misdirected, postage due or delayed mail, or for any incorrect information provided by you to Promo Process.
·   Limit ONE rebate per offer per person, household, family or address. This offer cannot be combined with any other offer.

·   This offer is void where restricted or prohibited by law.

·   Rebate checks are void if not cashed within 90 days of issuance and cannot be reissued. Rebate offer expires if the rebate
    check is not cashed within 90 days of the check date, after which time Promo Process will have no further obligation to

·   Claims that do not comply with the terms of this offer will be rejected.
