This statement provides information required by Arizona law about privacy,
confidentiality and related policies for individuals who use our official web site and other
electronic services.

University Medical Center (UMC) provides online information and services to staff,
employees and the public to supplement services provided on-site. We respect the
privacy and confidentiality of every individual who visits or uses our web site. We want
you to fully understand what information we gather from you, how that information is
collected, how that information is used, and how you can control the collection and/or use
of information about you.
This statement applies to all information collected by or submitted to UMC. It is not to be
construed as a contractual promise, so please read this statement carefully to ensure that
you understand precisely the applicable privacy policies and practices.


1. Personally Identifiable Information

There are cases in which we may ask you for personally identifiable information, such as
your name, mailing address, and e-mail address. For example, we may request personally
identifiable information for online shopping, transactions or other services that require
registration or subscription. We may also collect information from e-mail or other
correspondence that you send us. UMC will not collect any personally identifiable
information about you unless you provide it to us voluntarily, with the exception of your
computer's IP address. The IP address is an identifier unique to each computer or Internet
usage session.

When you do provide us with personally identifiable information, UMC may:
      Store and process that information to enhance your experience with our web page
      so that we provide a service that is relevant and easy to use;
      Use that information to contact you in order to inform you of our service
      enhancements or promotional activities;
      Disclose that information to third parties when we have reason to believe that
      disclosing this information is necessary to identify, contact, or bring legal action
      against someone who may be causing injury to or interference with UMC's rights
      or property, other members or users of this web site, or anyone else that could be
      harmed by such activities; and
      Disclose that information to third parties when we believe in good faith that the
      disclosure is required by law or a court of competent jurisdiction;
      Except as stated in this notice or as otherwise stated at the time that the personally
      identifiable information is gathered, we will not provide personally identifiable
      information to third parties.

UMC's web site may contain links to external web sites that you are able to access. The
existence of these links does not imply any affiliation, endorsement, or responsibility for
the privacy practices or policies of any site that you are able to access through these links.

3. E-mail and Form Information.

UMC does not obtain personal information about you when you visit our web site or
other online services unless you provide us that information voluntarily. If an individual
sends or responds to an e-mail message with a question or comment or completes a web-
based form, the form or communication may contain personally identifying information
that will only be used to directly respond to the requestor, unless otherwise stated
specifically. The request for information may be redirected to another part of the Medical
Center that may be in a better position to respond to the request. Any personal
information you provide will not be released to outside parties unless we are legally
required to do so in connection with legal proceedings, law enforcement investigations,
or state law.

4. System-Generated Information.

In addition to information actively provided by individuals using the UMC web-site and
other online services, we may record information such as but not limited to the following
types of information each time these access points are used:
Internet (IP) address of the computer being used
        Web pages requested
        Network software access
        Referring web page
        Browser used
        Date, time and duration of activity
        Passwords and accounts accessed
        Volume of data storage and transfers

UMC uses this information to monitor, preserve and enhance the functioning and
integrity of the system. Information is collected for analysis and statistical purposes, and
is used to help diagnose problems with the server and to carry out other administrative
tasks, such as assessing what information is of most interest, determining technical design
specifications and identifying system performance and/or problem areas. This
information is not used in any way that would reveal personal information to external
constituencies except as described above.

5. Monitoring.

In cases of suspected violations of UMC policies, especially unauthorized access to
computing systems, system administrators may take such measures as are necessary
including authorizing detailed session logging. This may involve capturing and retaining
a complete keystroke log of an entire session. In addition, a system administrator for the

equipment involved may authorize limited searching of user files on individual or
networked computers to gather evidence on a suspected violation.

6. Cookies.

Cookies are short pieces of information used by web browsers to remember information
provided by the user, such as passwords and preferences from past visits. Additional

UMC web pages may use cookies to store service information. Some web-based services
require cookies for access. Any information that UMC may store in cookies is used
exclusively for internal purposes only and as described above.

7. Alternatives to Electronic Transactions.

UMC provides online access to information and services for staff, employees and
members of the public. If you prefer not to provide any information online, you may
cancel the transaction and contact the administrative unit responsible for the service to
learn about available options for conducting business in person, by mail or by telephone.

8. Disclosure of Information.

UMC does not sell or distribute confidential information it collects online to individuals
or entities not affiliated with the Medical Center or the University or Arizona, except in
the very limited circumstances described below. Non-confidential information may be
sold or distributed pursuant to Arizona Public Records law.

        Public records law. Under the Arizona Public Records Law, UMC may be
        required to provide information in UMC's records to a third party.
        Contractors. From time to time, individuals or companies under contract with the
        UMC may have access to information in the course of the service they provide.
        However, those entities are not permitted to use or re-disclose that information for
        unauthorized purposes. No other entities are authorized to collect information
        through UMC's web site.

9. Security.

The confidentiality and security of all personally identifiable information associated with
our users is of utmost concern to us. Once we receive your personally identifiable
information, we will take reasonable efforts to ensure its security on our systems.
Unfortunately, although every effort is made to secure network communications, UMC
cannot ensure the absolute privacy of online communications over the Internet.
Individuals using online services must be aware that you do so at your own risk, and that
you must also take steps to protect personal information, such as closing the web browser

when finished using the site. Failure to do so may result in personal information being
viewed by someone else using the same computer.

10. On-line payments.

UMC accepts credit card payments on-line for a variety of goods and services. Unless
otherwise noted on the site, all credit card transactions are encrypted. Information entered
to complete a transaction will not be used by UMC for any other purpose unless the
purpose is described on the site.

11. Your Acceptance of These Terms.

         Please be aware that any information placed or stored in a University Medical
        Center Corporation owned or operator computer is subject to review by UMC at
        any time. By using this web site, you signify your assent to the terms and
        conditions of this notice. If you do not agree to this notice, please do not use this
        site.

        Please note that we may change the terms and conditions of our use policies at
        any time by posting revisions on the Service. You agree to review the terms and
